• To begin speech services with Butterfly Speech Therapy through Kaiser, you must already have a completed evaluation through Kaiser 

• Once you receive your approval letter for outside provider care, you can reach out to Butterfly Speech Therapy to schedule your start date

• You will need the following information to provide to the clinician: Patient's Full Name, Birthdate, Kaiser Medical Record Number, and information about Co-Pay or Deductible
